The holder can't be eliminated? Do the others know who has it at the time of voting? How do they decide who gets it? Is it just whoever has it in their possession at the time?

After they vote, but before they get read out, the HII holder must present it to avoid being voted out. So if they play it wrong they can waste it. Or alternatively leave with it in their pocket.
