Let me tell you about my first day of clinical on the ward.

RN asks me to help a 68yo man with Downs Syndrome with a bottle, due to the fact he was on 02 therapy for pneumonia.

I lift his hospital gown and I am faced with 3 balls, no cock. I ask him to help me, but he just grins, so I pick up the top ball and sort of squeeze it in the bottle. I ask if that is okay? He just kindly nods and continues to grin. Nek minnut I feel warmth down my leg and with horror I realise I have his ball in the bottle and he is pissing on my foot.

I spent the rest of my shift walking with a squishing piss filled shoe.

You will be fine. Don't undertake any task you don't feel 100% confident with. Listen, watch and learn. And welcome to the nursing trenches.

Develop poker face asap is my best advice, and also always ask for help if don't feel confident yet

If you are doing your placement in a nursing home, please remember to knock on room doors first, then introduce yourself. It is my pet hate when nurses burst in an commence cares without consideration which in essence is a breach in patient privacy.

Some day one tips with client care

1. Wash hands
2. Consider skin integrity, check on areas under breasts, groin, sacrum and stomach apron if overweight
3. Wash hands
4. Check heels and watch diet and fluid intake on non amubulent clients
5. Wash hands
6. Smile lots
7. Wash hands
8. If you are struggling to make chit chat, talk about pets
9. Wash hands
10. Report any concerns immediately to your RN or EN

Most importantly, have a super day!!!!!

I am not sure if I did.

I had lone late night drunken ramblings. Peen was a straight up, here I am, I don't give a fuck, punch in the face, kind of guy. The inappropriateness of his method when communicating stood out like dog balls. The subtly in the spite which is served to some of you is what effects me. The way he turned Jessy's shared "get a job have a baby" into an anonymous rort or OK dilemma. Try's to use Bleachy's faith against him, you with your Gran, it is just so spite ridden.

I honestly think the way he subtly attacks you affects me more than it does you! I swear to Bleachy's God, I would rather he aim it at me than you guys!

Yeah I wondered if that was aimed at me or not. If he didn't take his passive aggressive approach and actually directed his concerns at me then I would have been able to explain to him that there is zero rorting or more dilemma stuff going on as the way I plan to reproduce does not qualify me for maternity leave. But that might be too logical and concise for him anyway. Crap now I'm doing the same thing to him.... oh well....

Yeah this week is in a nursing home and next week is in a hospital. I'm more nervous about the hospital, but I guess it is good that I have the nursing home first, so I can put into practice all the basics and figure out where I need to improve on.

Nursing home is a great starting point for you.

Prepare to be shocked, and prepare to cry. You do become somewhat desensitised to the suffering of clients as time goes on.

For my first month of placement I would literally dry retch all the way in, and cry all the way home.

My first week of placement had me wondering if I could even do it. I mean what sort of nurse can't work out a cock from a ball?! The more you learn and grow, the more you understand anatomy and physiology, the more the circle of life makes sense, even if it can be heartbreakingly cruel at times.

Come back this arvo and debrief. You will soon see, debrief is the official term nurses use to describe gossip.

So I call my injector last week for an appointment for today, but she is fully booked till next Friday week (pre Christmas rush). Devastated I make the appointment for next Friday week.

Last Friday I get a phone call asking me if I could reschedule my Monday appointment from 09:00 to 11:15. Confused I ask how this is possible because my booking is Friday week. Turns out "someone" made the appointment THREE MONTHS AGO!!!!!

Sure it could have been me on my last appointment there as I do have a habit of booking in advance or did I....?

Nursing homes are the best way to start they help with getting time management under control (due to more pt load but less acute) and help with building rapport
 
And the smells

Oh the smells! When I concentrate I can make myself smell a pseudomonas wound! Once you smell it you can never unsmell it!!

I tell you what I still struggle with. Sputum and vomit.

One of the most confronting things I have seen was a trachy that had maggots in it!!!! Flies had laid eggs at the base!!!

Fresh post op stomas! Never ever unsmell that, but don't worry @Inigo Montoya you get used to them

Yep Sputum is one thing I never could get used to. YUCK re trachy maggots, must of been clean as a whistle though!
