Also, one of my mates had to drive his younger brother to the hospital a few weeks back because he had testicular torsion at about 4am. Apparently they have to operate on that shit quick, or you can become infertile.

In general, not very. Once you're out of puberty, it's even less common. It has something to do with how they grow, I think. Basically imagine the nut tube tying a knot on itself, or around the nut. It's enough to give you ghost pains just thinking bout it.
